Reporting Sexual Abuse

To report an instance of sexual abuse by clergy, religious or laity affiliated with the Diocese of Rockford:

If you are a Mandated Reporter, you must fill out the CANTS 5 form after you make a report to DCFS only if they tell you an investigation will pursue.

*All Employees and Volunteers of the Diocese of Rockford, that are considered to have contact with minors, sign the Mandated Reporter Form (CANTS 22) and are Mandated Reporters.

How to Report Abuse to Vulnerable Adults – Notify Illinois Adult Protective Services:

   To Report Abuse

  • Anyone can report a case of abuse.
  • To report suspected abuse, neglect, exploitation and self-neglect of an adult age 60 or older or a person with disabilities age 18-59 call the statewide, 24-hour Adult Protective Services Hotline: 1-866-800-1409.
  • For residents who live in nursing facilities, call the Illinois Department of Public Health’s Nursing Home Complaint Hotline: 1-800-252-4343.
  • For residents who live in Supportive Living Program (SLP) settings, call the Illinois Department of Healthcare and Family Services’ SLP Complaint Hotline: 1-844-528-8444.
